Ozempic is a medicine originally developed to treat type 2 diabetes.
Authorised across the EU since 2018, it helps regulate blood sugar levels by influencing insulin production.
In recent years, however, it has become widely known for another effect – weight loss.
Many patients lose weight while taking it, which has helped drive its popularity on social media as a potential alternative to traditional dieting.
There are also other, similar medications available in Germany, including Wegovy, Saxenda and Mounjaro, some of which are specifically approved for weight management.
What’s the law on Ozempic in Germany?
Ozempic is legal but strictly regulated in Germany.
Crucially, it is only officially approved for the treatment of type 2 diabetes, not for weight loss.
If a doctor prescribes Ozempic for weight loss, this is known as off‑label use, meaning it is being used outside its authorised indication.
Doctors in Germany can prescribe medicines off‑label in certain cases, but they are expected to follow medical guidelines and consider approved alternatives where available.

Can you buy Ozempic without a prescription in Germany?
Like other similar drugs, Ozempic is prescription-only in Germany, which means you must have a valid prescription from a doctor in order to obtain it from a pharmacy.
Pharmacies are not allowed to dispense Ozempic without a prescription, regardless of whether it is being used for diabetes or weight loss.
Will a doctor prescribe Ozempic for weight loss?
Because Ozempic is not approved for weight loss, doctors in Germany will often prefer to prescribe medicines that are specifically authorised for obesity, such as Wegovy.
To qualify for these treatments, patients typically need to meet medical criteria such as:
- a body mass index (BMI) of 30 or above, or
- a BMI of 27 or above with a related health condition such as high blood pressure or type 2 diabetes
Typically, doctors in Germany will assess whether lifestyle changes or other treatments have been tried before prescribing medication.

Does German health insurance cover Ozempic?
Generally, statutory public health insurance does not cover weight‑loss drugs.
Under German law, medicines used primarily for weight reduction are usually classified as “lifestyle” treatments and are excluded from reimbursement.
This position has been reinforced by court rulings, including a 2025 decision by the Mainz Social Court, which found that weight‑loss injections such as Wegovy are not covered by statutory insurance.
But if Ozempic is prescribed for its approved use – type 2 diabetes – it may be covered.
People with private health insurance may be able to claim costs in some cases, depending on their policy and whether treatment is considered medically necessary.
How much do weight-loss drugs cost in Germany?
If you are prescribed Ozempic or a similar drug for weight loss, you will usually have to pay for it yourself.
Prices vary depending on the medication and dosage, but they typically range from roughly €150 to €450 per month.
Costs can be higher for certain drugs or higher doses and there could conceivably be some issues around supply.
Germany, like other countries, has experienced shortages of Ozempic in recent years. High demand – particularly linked to its use for weight loss – has created pressure on supply, and authorities have emphasised that the drug should be prioritised for people with diabetes.
